Submit your affidavit to AriseAffidavits@arise.com and CC hr@bynesco.com. The Subject line must include the Client Program name and your CSP ID.
If you need to check the status of your Affidavit, please use AVA in the Arise Portal (portal.arise.com) to be connected to a live chat support specialist.
